Seth Brundle (The Fly Series) had became a fly mutant because due to a failed teleportation experiment when a Fly had hovered into the device, thus causing him to slowly mutate overtime.
Fusion Zamasu (Dragon Ball Super) despite being equally powerful thanks to fusing with Goku Black, had gained a form known as Half-Corruption, which as its name implies, the half of his body had been almost genetically mutated into purple slime.
